Sign it and mail it into the address for Secretary/Treasurer Timothy O'Neal (address on form). For Brigade members, include a check or money order for total of $30 (includes $15 for application fee and $15 for first year dues). For Grant's Guards members, include a check or money order for total of $10 (includes $5 application fee and $5 for first year dues).
